New Student-Athletes/First-Years/Transfers
To be completed by Doctor
- Physical Form - Student-athletes must hand in a copy of a physical completed no more than 6 months prior to the start of their season.
- Sickle Cell Disclosure Form - The NCAA now requires that all newcomers have to complete sickle cell test (newly passed legislation effective 8/1/22)
- It is recommended that student-athletes reach out to their primary care physician or the hospital they were born at to see if they already have their test completed.
- Student-athletes need to submit either the test result or the disclosure form completed and signed by their doctor.
- Complete your ADHD Form (if necessary)
To be completed by Student-Athlete (or Parent/Guardian if under the age of 18):
- Complete your Medical Packet
- Read the Student-Athlete Fact Sheet

- Complete your Concussion Statement
- Create your Online Profile - Directions (if needed)
Forms can be dropped off in the Sports Medicine mailbox in the athletic center main office or mailed to:
